Once the form is complete it must be uploaded to your secure account at MotorsportReg.com. Instructions to do this can be found at the bottom of the form and the bottom of this email. Please do not email these forms.

Upload instructions are – At Motorsportreg.com select My Account, click on your Profile, under Club Membership edit POC – upload the form and update the medical date.
